When You Leave

Discharge Planning

Discharge planning helps to ensure that when you leave hospital, you do so in a safe and informed manner. Murray Valley Private Hospital takes a multidisciplinary approach to discharge planning and we recognise that patients and carers are central to this process.

We recommend that you consider how you will manage at home after discharge.

  • Will you require someone to stay with you after being discharged?
  • How will you prepare food, do the housekeeping and / or shopping?
  • Will you need any special aids?

Staff will assist you with planning and arranging care needs through community based health and support services after discharge. Your doctor will discuss with you when he / she thinks you are well enough to go home.

Your nurse will talk to you about any special services or equipment you may need after you are discharged.

Discharge Time

All hospital inpatients must be ready for discharge prior to 10:00am. If you are unable to be collected before 10:00am staff will escort you to our patient lounge where you can comfortably wait until you are collected. We encourage patients to arrange their discharge transport with family or carers prior to admission if possible and at least a day prior to discharge.

Discharge Summary

A summary of your illness, your treatment and care will be written by your doctor for your general practitioner. A copy of your ongoing treatment, appointments and medications will be given to you.

Feedback

A patient questionnaire will be available in your room. Its purpose is to assist the hospital to review and continuously improve the quality of service. Comments are strictly confidential and you are encouraged to complete the form. The hospital’s executive team are available, should you wish to discuss any issues which may arise during your stay.

Outpatient

If you require an outpatient program at Murray Valley Private Hospital staff will contact you at home regarding your appointments if these have not been made prior to your discharge.

Appointments

Appointments with your specialist will be arranged by the nursing staff.

Accounts

We request that patient accounts are settled on or prior to discharge. Payments can be made via cash, cheque, credit card or EFTPOS at Reception.
